Instructions for submitting a veterinary diagnostic sample:
1. Download the forms below. Page 1 includes collection and shipping instructions. Page 2 is the sample submission form.
SAMPLE SUBMISSION FORMS (pdf)
2. Fill out page 2 (Diagnostic Real-time PCR Sample Submission Form) and include it inside your shipment.
3. Make sure all of your samples are in tightly sealed tubes or containers. Example: drippy poop in a glove is not a sealed container!!!
4. Ship the samples OVERNIGHT (FedEx or UPS) to the following address.
